For those who do not want to download this huge file, there is a link to
get a CD-ROM of the fixpack at

ftp://ftp.software.ibm.com/ps/defect_submission/ptfform.txt

This gives you a text file with all the information you need to fill out
and send in to IBM via  e-mail, fax, or snail mail.  It looks like it
would be a free service less media cost and shipping.
