 Not much fixed in this version over ib102. ib104 was an increment of 
 build number while I reintegrated the changes from ib099 to ib103 into
 a restored copy of the source.
 
 1) Add checking in SaveHeaderData for malloc success. May stop crashes
 when saving newsgroups. 
 2) Change max filehandles to current + 255 on startup. Hopefully this 
 will stop file open errors.
 3) Release newsgroup semaphore occasionally to stop lockouts
 4) Backout locking changes in nntp.c to try to fix corruption
 5) Backout headers.idx file rewrites to see if this helps the 
 mysterious wandering server problems
 6) Fixed ugly dashes in 640x480 for selected article
