A W-4 form is the Employee's Withholding Allowance Certificate form, which is used by employees to change their federal and/or state withholding allowances/marital status so their employer can withhold the correct Federal income tax for their pay. Changes may be made to the W-4 information at any time during the year by submitting a new W-4 form. Employees can obtain an estimated dollar amount of what the Federal Tax withholding would be when they change their marital status and/or withholding allowances by using the Federal
IRS Withholding Calculator. Employees may submit a Federal W-4 form electronically or print out a hardcopy W-4 form and mail to
U.S. Payroll Services for processing. Faxed State or Federal W-4's will not be accepted. Federal and State W-4 forms can be accessed from
Forms & Apps on the

To initiate a change to your Federal withholding (exemptions, marital statis, additional withholding amounts), you must submit a new
Federal W-4 form. Any Federal W-4 form submitted online via the Web is a full replacement of any prior submission. You may change your tax withholding allowances as many times as you wish during the year. There is no limit. The form has audits to validate your selections, helps eliminate problems incurred with hardcopy mailing, and enhances the overall timeliness of changes to your federal tax withholding.
To change your state withholding allowances, you must submit either your specific
State W-4 form or the generic "State/City/County Employee's Withholding Allowance Certificate" (form #GM02-4442). Please be sure to include your employee serial number on the federal and/or state form.
